How to schedule

New Patients: 

  • Visit the Find a Provider page to see which of our providers are accepting new patients.
  •  After you select a provider, please call the clinic where they practice to confirm your insurance plan is accepted and set up a new patient appointment.
  • New patients must call the clinic to schedule a new patient appointment. This allows staff to verify your insurance and make sure your information is accurately entered into our electronic health record.

Established patients: 

Established patients can schedule via phone or online through a Family Care Network MyChart account. If you do not have a Family Care Network MyChart account, you can register for one online.

When scheduling online:

  • Not all appointments are suitable for online scheduling. This includes procedures, new patient appointments, and children’s well exams.
  • If you don’t see the appointment type you are looking for, please call the clinic.

All Family Care Network clinics reserve same-day appointments for unexpected health issues. To schedule a same-day appointment with an available provider at your clinic, please call your clinic.

Important FAQs

What types of appointments can I schedule through MyChart?

Established FCN patients can schedule a variety of appointments through their Family Care Network MyChart accounts, including follow-up appointments for ongoing issues, annual well exams for adults, and appointments to address mental health concerns.

Please do not use the online scheduling tool to schedule medical procedures, same-day appointments, or appointments with our Laboratory Services department, including blood draws. Lab draws can be scheduled here: Lab Scheduling

How do I schedule a telemedicine appointment?

Telemedicine appointments can be scheduled by phone or online through MyChart. Learn more: Telemedicine

Family Care Network also offers an On-Demand Telemedicine option for urgent care. Patients are seen on a first-come, first-served basis. Learn more: On-Demand Telemedicine

How do I schedule a lab draw appointment?

Established FCN patients and non-FCN patients can schedule an appointment at any Family Care Network Laboratory Services station at fcnlab.as.me.

How do I schedule an OB appointment?

Established Family Care Network patients can call their normal clinic to discuss scheduling for pregnancy care.

How do I schedule an appointment with Sports Medicine?

To schedule an initial evaluation with our Sports Medicine physicians, please call the clinic at (360) 756-0382.

Follow up visits may be scheduled through a Family Care Network MyChart account.

How do I schedule an appointment with Behavioral Health?

Family Care Network’s Behavioral Health program is for established FCN patients only, by referral from their primary care provider. If you have been referred for a Behavioral Health appointment, please call (360) 483-5231 for assistance.
