Making Care Primary

Family Care Network is participating in a new primary care payment model program developed by Centers for Medicare & Medicaid Services (CMS), called Making Care Primary. The 10.5-year pilot program began in July 2024.

The Making Care Primary (MCP) model provides primary care clinicians with enhanced model payments, tools, and supports to improve the health outcomes of their patients. This model will attempt to strengthen coordination between a patient’s primary care clinician, specialists, social service providers, and behavioral health clinicians, ultimately leading to chronic disease prevention, fewer emergency room visits, and better health outcomes.

The first two years of the program focus on “behind the scenes” work, such as data analysis and workflow development, to create an infrastructure that can support advanced primary care services. It is important to note that this program does not change or reduce Medicare benefits.

As part of this program, Medicare will share some of your personal health information with FCN, such as when hospital, emergency, or specialist care is provided. This will help provide us with a more complete picture of your health and allow us to better coordinate your care.

What you can do to help

When you let Medicare know that you have an FCN primary care provider (PCP), the MCP program provides access to tools and services that will help your provider coordinate your care and improve your health.

You can update your PCP with Medicare by visiting Medicare.gov and adding your PCP to your favorites list.

Important FAQs

Does FCN's participation in MCP change or reduce my Medicare benefits?

No. Medicare benefits are not affected by our participation in the program.

Why was this program developed?

This is a new payment model. Medicare’s attempt to shift funds, through a new payment model, to support what research has shown, that primary care can improve health outcomes.

Who is participating in the pilot program?

Organizations in the following states can participate in the pilot program: Colorado, Massachusetts, Minnesota, New Jersey, New Mexico, New York, North Carolina and Washington.

Can I opt out of the program?

MCP is a special contracting agreement between FCN and CMS, and Medicare beneficiaries are not technically able to “opt out”. However, Medicare beneficiaries can request that their data is not shared through the MCP program by calling 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.
